1. Complete the following form. This form will ask you biographical information and also require you to upload the follow:
   1. Signed Contract (you should have received this from your host)
   2. A CV or Biographical Statement
   3. A headshot when appropriate
   4. An image of your work/practice when appropriate
   5. Appropriate Tax Form:
      1. W-9 Form for US Citizens and Permanent Residents
      2. W-8BEN for International Visitors
   6. Appropriate Payment Form:
      1. We can issue electronic payments via ACH for visitors with a US bank account. Fill out this form if this applies to you
      2. International visitors without a US bank account can be paid via Wire Transfer. If this applies to you, here is more information
      3. If you choose none of the above, a physical check will be issued to you

Example Biographical Statement:
Laura Mott is the Curator of Contemporary Art and Design of the Cranbrook Art Museum in Bloomfield Hills, Michigan.  Previously, she worked at various galleries and institutions in Europe and the United States, including the University of Gothenburg, Sweden; Gothenburg Konsthall; Iaspis, Stockholm; Mission 17, San Francisco; Peter Freeman Gallery, New York City; and the Whitney Museum of American Art, New York City.  She received her BFA/BA dual degree in Art History and Fine Art from the University of Texas, and MA in Curatorial Studies from Bard College.

Example Lecture Summary:
In 1950, Time magazine ran a story referring to Jackson Pollock’s painting as “chaos” – prompting Pollock to telegraph the angry reply: “NO CHAOS DAMN IT!” Using our current exhibition The Islands of Benoît Mandelbrot: Fractals, Chaos and the Materiality of Thinking as a starting point, Halsall will consider Pollock’s claim and offer speculations as to what theories of systems, chaos, and complexity might mean for understanding nature, art, and aesthetics.
